PNH and Ahus Market

The future of the global pnh and ahus market looks promising with opportunities in the hospital, specialty clinic, and research institute markets. The global pnh and ahus market is expected to reach an estimated $12.2 billion by 2035 from $7.4 billion in 2027 with a CAGR of 6.4% from 2027 to 2035. The major drivers for this market are the increasing prevalence of rare blood disorders, the rising demand for targeted therapies, and the growing awareness of PNH and aHUS.

  • Lucintel forecasts that, within the treatment type category, eculizumab is expected to witness higher growth over the forecast period due to well established efficacy of eculizumab in clinic, long-term experience.
  • Within the end use category, hospitals is expected to witness the highest growth over the forecast period due to increased number of inpatient diagnoses of rare complement-mediated diseases.
  • In terms of regions, North America is expected to witness the highest growth over the forecast period due to capacity for rare disease diagnostics and reimbursement.

Emerging Trends in PNH and Ahus Market

From 2025 to 2027, the PNH and aHUS markets will focus more on diagnosis and treatment rather than on rescue therapy. More competitive pipelines will lead to a better inhibitors and more convenient treatment options. Continued limitations on reimbursement and availability will impact the use of these therapies. It will be best to analyze markets based on individual diseases.

  • Precision Diagnosis: There will be increased use of next-generation gene sequencing and complement pathway testing to help in the evaluation of patients with aHUS. Flow cytometry will still be essential for diagnosis of PNH. With more precise diagnosis of these diseases, especially in the next 3-5 years, patients can be directed to the most appropriate therapy.
  • Long-Acting Therapy: There will be a focus on convenient and less frequent therapy for PNH starting in 2024 with the approval of Crovalimab. Other therapies targeting C5 will also be approved and will impact the convenience of therapy for PNH.
  • Oral Therapy: There will also be a focus on convenience for aHUS and PNH with the approval of Iptacopan in 2023/2024. Other oral therapies will also be approved and will impact the use of intravenous biologics for these diseases.
  • Outcome-based Access: Payers have expanded their measure of success to include renal outcomes and to what extent aHUS treatment reduces the need for dialysis. Reducing the length of hospital stays and avoiding transfusions and renal replacement therapy remains the key measure of success.
  • Supply Chain Considerations: Medicines to treat aHUS remain extremely costly. The large upfront costs of this therapy discourage payers to cover aHUS treatments and encourage a thorough assessment of the value of each course of treatment. During the next five to seven years, aHUS drug manufacturers will focus on regional rather than global coverage.

In the next five years, treatments for both AHUS and PNH will become less burdensome as complement inhibitors become longer-acting and greater dosing flexibility is achieved. Competition will increase as the pharmaceutical industry advances oral complement inhibitors. The greatest opportunity for market differentiation will occur through the creation of a reliable drug supply combined with evidence of improved patient outcomes supported by the engagement of the target specialist medical practitioners.

Recent Developments in the PNH and Ahus Market

2025 to 2027 will see increased competition in the PNH and ahus markets with the introduction of oral complement inhibitors, long-acting antibodies and other targeted therapies. Also expect changes in the manufacture and regulatory approval process to allow for different methods of administration and to cover larger geographic areas. Because of these changes Lucintel believes the first manufacturers to gain approval will face competition from companies that control breakthrough hemolysis and other kidney and adverse outcomes, as well as reduce the treatment burden.

  • Regulatory Expansion: The first of likely many changes to the PNH market will occur with the launch of crovalimab. Approved in May 2025 by the U.S. Food and Drug Administration (FDA) for the treatment of adults and children 13 years of age and older with PNH, it will allow many patients to reduce the frequency of their C5 therapy to once a month.
  • Oral Therapy Adoption: Novartis's iptacopan, the first of the oral inhibitors of factor B, will likely alter the PNH treatment market when it gains approval. Once launched, doctors will have the option of presaging a medication that, compared to the other therapies available, will increase patient autonomy. Whether patients will be able to make their own treatment decisions will depend on each individual's ability and willingness to comply with the treatment and monitoring regimen to limit the risk of thromboembolic events.
  • Subcutaneous Competition: Adding to previous competition for the treatment of complement incomplete blockade with Pegcetacoplan (Pegcetaco) and Danicopan (Danicopan) in 2025, additional participants entered the market with add-on therapies for complement blockade for the treatment of aHUS and other complement-mediated disorders. While companies generally targeted the treatment-naive population in 2025, they started to position their products for the control of refractory cases in 2025.
  • Rare Disease Access Programs: Companies and patient organizations focused on expanding access to reimbursement programs for rare diseases in both developed and emerging markets in 2025. Improved diagnostics and more targeted therapies resulted in an enlarged treated population. Given the high unmet need for these therapies, companies focused on building the body of evidence to demonstrate that the therapies improve the quality of life of patients in order to gain market access.
  • Product Pipeline Expansion: Current therapies for both PNH and aHUS are biologics and, therefore, have a limitation of long-term control and pose financial burden on the health care system and patients. In 2027, improvements in diagnostic and therapeutic care for both diseases are anticipated, including the use of gene therapy and other novel therapeutic approaches.

The value addition to patients will be the key driver for growth of companies operating in the complement inhibitors market. Companies with strong product supply, more convenient dosing, and renal outcome data are likely to dominate the market. While the breadth of the market is expected to improve, the bar will still be set high for companies to improve outcomes.

Strategic Growth Opportunities in the PNH and Ahus Market

Treatment of complement-mediated conditions is evolving to provide longer-term control and moving beyond the sphere of specialist therapies. The next few years (2024-2026) will witness the launch of subcutaneous therapies, better diagnostics, and payer trend-based shifts to broader reimbursement. According to Lucintel, revenues from products beyond the established C5 inhibitors is likely to pick up in areas where access and/or reimbursement is restricted to the PNH and aHUS patient populations.

  • Earlier-line Treatment of PNH: The International expert consensus on the diagnosis of PNH, released in June 2025, recommended that hemolytic and thrombotic complications be investigated in all patients with PNH. This will facilitate earlier diagnosis and treatment, and potentially reduce the morbidity and cost associated with the disease.
  • Oral and Subcutaneous Products: The availability of oral and subcutaneous PNH therapies will reduce the need for lifelong dependence on i.v. therapies. In February 2025, the FDA approved Fabhalta to treat PNH. The convenience of this and other subcutaneous and/or oral therapies will increase treatment adherence and potentially improve patient outcomes.
  • Atypical Haemolytic Uremic Syndrome Expansion: The approval of eculizumab by payers for renal aHUS will increase its use in other centres, and reduce the demand for plasmapheresis. The improved reimbursement will allow complement blockade to be administered earlier in the course of aHUS.
  • Biomarker-led Care: In September 2025, research focused on complement biomarkers and other renal and hematologic endpoints. Separately, clinical testing will be a profitable service. It is expected that with molecular and other pharmacodynamic data, health care providers will make treatment decisions and manage patients.
  • Access in The Developing World: In April 2025, the WHO began procuring rare disease therapies with over 100 member countries. It is expected that in the following five years, using strategies such as regional collaborations, and pharmacovigilance, and price protection, complementary therapies will be available to a larger market.

Efficiency and quality will differentiate market leaders. Manufacturers and service providers who integrate and offer complementary therapies will succeed. Ultimate outcome and long-term improvement will be the end goal. The best market players will innovate to offer local solutions to improve patient access while not compromising patient safety.

PNH and Ahus Market Drivers and Challenges

The PNH and aHUS markets are impacted by various economic, regulatory and technological factors. Some market drivers include rising disease awareness, development of new compliment-based therapies, improved diagnostics, and increased reimbursement. Some market restrictions include high costs of treatments, limited number of specialists, and complexities in administration. According to Lucintel, the long-term outlook and business strategy of market players is governed by new product development and patient centredness.

The factors responsible for driving this market include:

  • Improved Awareness and Diagnosis: Increased public awareness and medical education efforts have improved diagnosis of PNH and aHUS. Additionally, several clinical studies are being conducted for compliment based disorders. Thus, it can be expected that in the next five years, improved diagnosis, awareness and referral systems will benefit and facilitate treatment of PNH and aHUS patients.
  • Broader Options: Traditionally, treatment for complement disorders has been infusion-based. However, new innovation has resulted in longer-acting C5 inhibitors, as well as other oral and inhibiting therapies. June 2024 saw the FDA approve the first subcutaneous treatment for PNH, further advancing the available treatment options. In the following 3 to 5 years, therapies targeting oral delivery and more effective control of EV hemolysis will further advance the treatment landscape.
  • Positive Trends in Pharmaceutical Law: Positive amendments to the regulation of orphan drugs, as well as more favorable views toward complement disorders, have resulted in a more positive environment for drug development. In February 2025, the EU extended the duration of market exclusivity for orphan drugs to 10 years. Positive trends in pharmaceutical law will enable the development of innovative therapies for aHUS and PNH.
  • Increased Availability of Appropriate Diagnostics: The introduction and advancement of multiplex laboratory testing platforms, complement and flow cytometry-based testing, and improved genetic testing have allowed for better distinction of atypical PNH and aHUS. In January 2025, improved diagnostics allowed for wider availability of specialized testing. Better and more timely diagnosis of these diseases will lead to increased use of specific pharmaceutical therapies.
  • New Innovations and Ease of Use: Longer dosing intervals, user-administered (self) techniques, pills, and therapies avoiding breakthrough hemolysis/incomplete control of the target condition are some of the recent advancements in research. In September 2025, ongoing studies to create next-generation complement medicines, targeting rare hematologic conditions, continued. Newer, more convenient and easier to use (injectable or pill) products will likely become available in the next 3-5 years, and increase the competition in the market.

The challenges facing this market include:

  • High Costs of Treatment: Complement inhibitors are biologics that are difficult to produce, and therefore, very expensive. In addition, due to the small patient populations, many drugs are not profitable to produce. Consequently, manufacturers charge extremely high prices for these medicines. In 2024/2025, prices for rare disease drugs were reported to be between $200,000 - $500,000 per year. During the forecast period (2019-2026), there was, and likely will be, a growing emphasis on cost-control, especially in developed markets. Consequently, reimbursement may be delayed or denied, and the use of alternative medicines (i.e., biosimilars), or prices based on the outcome of use (value-based pricing) may increase.
  • Incomplete Diagnosis and Access: Due to similar symptoms, some rare conditions, like PNH and aHUS, may be confused with other, more prevalent conditions, like hematologic or kidney disorders. Diagnostic and specialty care (i.e., hematology and nephrology) shortages may limit the market, particularly in undeveloped and rural regions.
  • Safety, Resistance, and Therapy Complexity: As with other biologic therapies, there are concerns with complement blockade therapies including the potential for adverse events and infection. Additionally, there remains the need for continued vaccination, and the strict monitoring of patients. Therapeutic blockade of the complement cascade may result in breakthroughs of infectious diseases including sepsis. The clinical studies currently underway (expected completion date October 2025) will further evaluate other pathways of the complement system. While the studies help to further elucidate safety concerns, the considerable uncertainty surrounding response and prolonged safety of the treatments may limit widespread use.

Complementary treatment of rare diseases is likely to increase, particularly with improvements in scientific, diagnostic, and therapeutic complementary measures. The current onerous nature of treatment, cost, safety, and inequity in coverage and reimbursement of rare diseases will continue to limit treatment. Improved therapy may, however, ease some of these constraints. Established complement inhibitors will likely face price competition. In the next three to five years, improved therapy may foster a greater balance between clinical benefits and convenience.

Country Wise Outlook for the PNH and Ahus Market

The market for PNH and aHUS drugs is experiencing changes including longer-lasting drugs that can be taken orally. Additionally, the market is expected to change in the next few years with more drugs covering rare diseases and greater convenience in drug dosing. Among these changes, Lucintel expects the biggest competition in the market will be in the area of drug price justification, positioning and access.

  • United States: The FDA approved Roche's drug, crovalimab (Piasky), in June 2025 to treat PNH. With this approval, Roche became the first company to gain a maintained drug approbation for PNH. The approval is expected to spur competition to develop drugs that need to be administered even less frequently, as well as to gain a larger share of the market.
  • China: Roche gained approval from China in August 2024 to market crovalimab to treat PNH. During the same time period, other biologics manufacturers in China increased their production of biologics. Because, until now, the majority of the treatments for PNH have been administered intravenously, the monthly subcutaneous administration of crovalimab is expected to increase the use of the drug.
  • Germany: Having received marketing authorization in June 2024, crovalimab is expected to be launched in Germany prior to other EU countries. Having an important manufacturing plant in Germany, and with EU approval expected soon, Germany will play an important role in the adoption of PNH therapies in Europe.
  • India: In February 2025, the Union Budget of India allocated 20 billion INR for setting up 3 Centers of Excellence (COEs) for Artificial Intelligence (AI) in the field of Healthcare. The COEs will help with the diagnosis and treatment of rare diseases in India. While the current investments do not directly impact care for complement inhibitors, it is expected that with improved diagnostic and medical care infrastructure in India, rare diseases such as PNH will be recognized earlier, and treated in a more timely manner.
  • Japan: In March 2024, the Ministry of Health, Labor and Welfare approved the use of crovalimab for treatment of PNH. It is expected that with this approval, other manufacturers will follow the lead of crovalimab and other subcutaneous C5 inhibitors for rare diseases in Japan.
